An inquiry has found "systemic failures" contributed to the deaths of two young people who took their own lives whilst in custody in a Young Offender's Institution, in Polmont, Edinburgh. 21-year-old Katie Allan was found dead in her cell in June 2018. Four months later, 16-year-old William Lindsay also died in custody. The inquiry has made 25 recommendations to try to prevent a similar tragedy.
